This policy explains how Heel & Beyond Podiatry collects and uses your personal information, and what your rights are. We are the data controller for the information described here.
What we collect
When you submit an appointment request, we collect the details you provide in the form:
- Your name, phone number and email address
- Your postcode and, if you provide it, your home address
- Whether you are an existing patient, and your preferred contact method
- Any preferred date and time, accessibility requirements and parking notes
- Any notes you choose to add about your concern
Information you give us about a health concern is treated as special category health data and is handled with additional care. Please keep the notes field brief. Detailed medical history is better discussed at your appointment than typed into a web form.
Why we use it
We use your information to respond to your request, confirm whether we can accommodate your location, arrange and deliver your appointment, and keep the clinical records required of a registered podiatrist.
Our lawful basis for handling contact details is legitimate interest and, where you have ticked the consent box, your consent. Our basis for handling health information is the provision of health care under Article 9(2)(h) of the UK GDPR.
How your request reaches us
The booking form is delivered by FormSubmit, a third party form processing service, which forwards the contents of the form to our email inbox. Your details pass through their systems in transit. We do not store your submission on this website, and this website does not use a database.
How long we keep it
Enquiries that do not lead to an appointment are deleted once they are no longer needed. Clinical records for patients we have treated are retained for the period required by professional standards for health records, and then securely destroyed.
Who we share it with
We do not sell your information and we do not use it for marketing. We may share information with another healthcare professional, such as your GP, where this is necessary for your care, or where we are required to do so by law.
Your rights
You have the right to ask for a copy of the information we hold about you, to have it corrected if it is wrong, to ask for it to be deleted where we are not required to keep it, to object to how we use it, and to withdraw consent at any time.
